Job Description:
A company is looking for a Care Coordinator Associate -
Authorization Hub.
Key Responsibilities
Assist in hospital transitions as part of an interdisciplinary care
team
Ensure smooth throughput and care transitions for patients
Utilize multiple data platforms and troubleshoot technology issues
as needed
Required Qualifications
Post-secondary coursework, preferably in a health-related field
Bachelor's degree highly desirable
Minimum of 2 years experience in a healthcare setting, preferably
as a Health Unit Coordinator or in a clinical role
Experience in utilization management, community resources, and/or
financial and quality issues affecting patient care desirable
